Responsibilities and Duties
This position will provide site monitoring services, conducting consistent on-site monitoring to assure that the people we support live a healthy and safe life. Monitoring will occur on an announced and unannounced basis, as driven by rules and regulations,
as well as agency procedures and needs assure the delivery of high quality care; take the initiative to be proactive in making any needed adjustments to ensure we are always progressive in our thinking and actions as we support the people we serve to live
a high quality life in a high-quality home and upholding our agency’s vision as a whole. Additional duties may include quality assurance documentation review.

Paid Training: You MUST be available for up to 40+ hours of PAID Formal Orientation. All Training is PAID and typically held Monday through Thursday 9:00 am-6:00 pm. Applicants must have the flexibility to complete our training upon hire.
HOURS/DAYS OF WORK CAN BE MONDAY-FRIDAY 8 a.m. to 6 p.m. Flexibility will be important. Management reserves the right to adjust schedules to meet the needs of the department.

Nurse Practitioner

Come join our team! We are seeking an LPN to join our unique and caring team here at Community Support Services, Inc. Our company is committed to high-quality standards for the people we serve. Our LPN will help ensure the best health care practices that are in line with designated state rules and regulations for people with Intellectual and Developmental Disabilities who live in individualized settings throughout the Denver Metro area. The LPN would attend our warm and welcoming office for 25 hours 3 days each week for 8.3 hours or similar. Examples of duties would include protocol development and training for individualized health needs such as g-tube training, epi-pen training, etc.; physician order coordination; medical appointment follow up; medication administration record review and follow up; hospitalization and discharge coordination; medical orientation training to personnel, and other. The LPN would join our Quality Assurance Team in the provision of state-of-the-art care for the people we serve

Currently Licensed Practical Nurse (LPN) in Colorado.

Compensation $22.00/hr. - $23.00/hr. (FULL TIME Employees 30+ hours per week, who choose our EXCELLENT company paid benefit package (after 60 days) would have an earning that equates to an additional $7.15 per hour, totaling $29.15 -$30.15 per hour). If you choose a 40 hour a week schedule you will earn $45,760.00/yr-$47,840.00
a year, plus $14,862/yr in benefits would make the annual salary equate to $60,622-$62,702 annually.

If you choose a 40 hour a week schedule you will earn $39,520.00/yr. or $41,600.00/yr. a year plus $14,862/yr. in benefits would make the annual salary equate to $54,382.00 or $56,462.00 annually.

HOST HOME PROVIDERS
Community Support Services, Inc. is accepting applications for Host Home Providers. This is an Independent Contractor position. In addition, we are currently accepting applications for Respite Host Home Providers. We look for individuals and couples who are sincere in their commitment and desire to care for and provide quality services and supports to the people we support.
